Is Povidone-Iodine Safe for Nasal and Oral COVID-19 Protection?

Can a common antiseptic be safely used to fight virus spread through the nose and mouth? This review examines the science behind povidone-iodine (PVP-I) as a nasal and oral disinfectant against coronaviruses.

Why PVP-I for COVID-19?

SARS-CoV-2 has been found in high concentrations in the nasal passages, nasopharynx, and oral cavity. Since these areas are major virus reservoirs, especially during early infection, they are prime targets for preventive disinfection.

Povidone-iodine (PVP-I) is a broad-spectrum antiseptic with strong activity against viruses, bacteria, and fungi. It's been used in surgery for decades and is now being evaluated for use inside the nose and mouth to reduce COVID-19 transmission, particularly in healthcare settings.

Key Safety Findings

  • Nasal use: Safe up to 1.25% concentration, for up to 5 months
  • Oral use: Safe up to 2.5–5% concentration, for 6 months
  • No adverse effects on:
    • Mucociliary clearance
    • Olfactory function
    • Thyroid function (in healthy individuals)
  • Allergy and contact sensitivity are rare

Protocols in Practice

Healthcare systems and hospitals have implemented protocols like:

  • 0.3 mL of 0.5% PVP-I in each nostril + 9 mL oral rinse (every 2–3 hours)
  • Use by both patients before procedures and healthcare workers
  • Some guidelines also support use in asymptomatic individuals, due to comparable viral loads

Effectiveness Against Coronaviruses

In vitro studies show:

  • PVP-I inactivates SARS-CoV and MERS-CoV in 15–30 seconds
  • Effective concentrations range from 0.23% to 1%
  • Although direct data on SARS-CoV-2 is limited, the high genetic similarity (>80%) makes it likely that PVP-I is effective

Safety Considerations

While generally safe, PVP-I use should be avoided or monitored in:

  • Pregnant patients
  • People with thyroid disease
  • Patients undergoing radioactive iodine therapy
  • Unconscious patients (due to aspiration risk)

PVP-I was associated with no reported aspiration pneumonia in awake patients, but caution is warranted during sedation or surgery.

Real-World Uses & Outcomes

  • Orthopedic and ENT surgery patients have used 5% PVP-I intranasally with reduced infection rates
  • Chronic sinusitis patients have used 0.08% PVP-I rinses for weeks without side effects
  • Long-term oral rinse use (up to 28 months) showed no irritation, no staining, no taste changes

Conclusion

Povidone-iodine is a well-tolerated, low-cost, and effective antiseptic that may play an important role in COVID-19 protocols, especially for:

  • Healthcare workers exposed to respiratory secretions
  • Patients before ENT, dental, or head/neck procedures
  • General use in COVID-19 hotspots or high-risk environments

Its safety in the nasal and oral cavities at proper concentrations is well-supported by both clinical and lab data. While more research is needed for SARS-CoV-2 specifically, current evidence supports its prophylactic use in reducing viral transmission.
